Introduction to the new Ethics Accountability Statement Process

Electronic Signing (via DocuSign)
Every member with an email address on file will receive an invitation from DocuSign to sign the Ethics Accountability Form. DocuSign is easy to use and works on different devices. Look for your email invitation on January 31. Can’t find your invitation? Email us at firstname.lastname@example.org.

It is preferred that members use the DocuSign option to complete the Ethics Accountability form. We are aware that some members may not have access to do so or prefer to complete a paper form. For those members, a printable form is available.
You can sign the form two different ways:
- Download and print the form on a standard printer, fill it out by hand, and mail it to the NACC: 4915 South Howell Avenue, Suite 501, Milwaukee, WI 53207
- On a computer or mobile device, fill in the portion of the form that allows you to do so (Name, Membership Number, and five spaces for your initials), then print the form on a standard printer, sign and date the form, and mail it to the NACC: 4915 South Howell Avenue, Suite 501, Milwaukee, WI 53207
Note that either way the form must be signed and dated by hand.

If upon reviewing the Ethics Accountability Statement you need to self-report an incident(s), please complete the form below and return it to the NACC National Office (by mail or email – email@example.com). The Ethics Commission will review the information and provide you further direction.
