1. To use the form, you must first download it from the NACC website. This is not a web-based form and cannot be filled out or submitted online; it must be downloaded and completed before being either emailed, printed and mailed in an envelope, or saved to a portable storage format (floppy disk, flash/pen drive, or CD).
2. To download the form, navigate to its location at www.nacc.org/certification/renewal.asp and right-click on the link for the Education Report Form. Choose “Save target as…” or “Save link as…” and save the form to the location of your choice on your computer. Note that in order to read and complete the form, you must have Microsoft Excel installed on your computer.
3. Now open the form from its saved location on your computer. When the document opens, a prompt will appear (see fig. 1). Click on “Read Only.” You will not need to enter a password.

4. The form will open. Type your personal information at the top and begin entering education report data. Note that you can only make entries in the correct spaces. All other areas of the form are protected against changes. Trying to change any other cell produces an error message (see fig. 2, below).

5. As you complete the form, a running total of your education hours will be automatically totaled, first as a sum for each area (personal, professional, and theological), and then as a grand total of all three sections. This grand total will appear in the lower right of the form.
6. If desired, you may print the Education Report Form after you’ve completed it.
7. To save the form, click File, then Save or Save As… Choosing “Save” will cause a message dialog box to appear (see fig. 3, below).

8. Saving the document you’ve edited can only be done if you save it under a new name. Give the document a name that identifies it clearly to you and click Save.
